Finding support for an individual with a developmental disability is easier when you have help from a case manager. Our Case Management programs are designed to assist you in finding and arranging beneficial developmental services as well as connecting you with other valuable services in your community.

Your case manager will help you advocate, make referrals and navigate local resources that are right for your unique situation. We support individuals who often need assistance to learn, understand or use information. We work closely with the individual we support, their family and others who are important in their life, to develop an Individual Support Plan, based on each individual’s needs.

Child Case Management

It’s what every child deserves and our case managers ensure children up to the age of 18 years with developmental disabilities or dual diagnosis receive adequate care, support, and access to a variety of fundamental services.

Our case managers work closely with other support agencies in the community and are caring, supportive and highly trained. We will work closely with you to make sure your child gets the most out of community services and the education system. Together, you will develop and move forward with a plan for the future as your child transitions into adulthood.

Adult Case Management
– Community Integration

Everyone in our society deserves dignity, respect and independence. Our Community Integration Services program is designed to help individuals who are 18 years and over, with a developmental disability or dual diagnosis, lead fulfilling, independent lives within a community setting. We ensure their rights and responsibilities are respected and protected. Our case managers begin by assessing each individual’s living situation, level of skill and degree of need, and serve as advocates for adults with developmental disabilities and work to ensure individuals they support have adequate access to:

  • Housing
  • Education
  • Work
  • Financial aid
  • Medical care
  • Legal advice

We will also be there to help individuals develop important life skills, such as budgeting, nutrition, travelling via public transit, and learning their rights and responsibilities. Increased independence is an achievable goal with the right help.
