Intensive Case Management

Comprehensive and thorough care

Adults, 18 years or over, with a developmental disability or dual diagnosis, serious medical condition, or a complex situation requiring more individualized and concentrated support, can find support and services with our Intensive Case Management program. It ensures that individuals receive the help they need to stabilize critical situations and find support within the community.

Our experienced case managers collaborate with all identified or required community services to stabilize the current situation and ensure ongoing supports are available. Often Individuals require support from a variety of services including mental health, corrections, health, and developmental services. We are here to help individuals make these connections and coordinate services to meet their needs and stabilize their situation.

This service is available to residents in the Brant, Haldimand Norfolk, Niagara or Hamilton regions, and the First Nation communities of the Six Nations of the Grand River and Mississaugas of the New Credit First Nation.
